REQUIREMENTS
Graduate of an accredited school of Physical Therapy. Registration in Minnesota and Wisconsin required. Membership in the American Physical Therapy Association. Current driver’s license and personal insurance must provide coverage while working for Lakeview Hospital Homecare. Continuing education as required by state licensing board. Ability to read, write and speak the English Language. BLS/CPR Required. Homecare Physical Therapy experience preferred.

DESCRIPTION
Responsible for planning and carrying out physical therapy assessments and treatment procedures in home with patients and their families,
